Referrals have become increasingly complicated. The following information is vital for helping us make timely referrals for your child and to make sure that you will not be receiving an unwanted bill.
Name and date of birth of the child being referred.
PCP (primary care provider) for your child. This is the provider you chose through your insurance, not necessarily the provider your child normally sees in the office or who made the referral.
Insurance company name and policy number. Please update this information every time you come to our office. If we make a referral with incorrect information, you may be responsible for the bill by the consulting physician.
The doctor’s name your child is being referred to. You may not know this until you make the appointment. Please call us with the following information:
a. Name, address, phone number, and fax number of the
